The following forms can be used to keep your records up-to-date at FlexRN.

• General Safety and Orientation
• Health Statement
• TB Questionnaire
• Evaluation (2 current evaluations are needed on a yearly basis)
• Drug Screen (Unfortunately, we are not able to have this available on our website however you may contact our Compliance Coordinator for details on this yearly update)
• PPD-Must be updated once a year/No Form for this. Please send in documentation of your PPD.
• Yearly self evaluation

Print and fill out the skills checklist(s) that best fits your area or areas of expertise. You must have a minimum of one year experience to staff as an agency nurse in any particular specialty.
